I understand that the course for which I have registered is the first step in a multi-stage process for obtaining certification from the state agency and that I am fully responsible for completing this process. I am aware that this process includes filling out state agency form(s), submitting the form(s) to the state agency, paying associated fees, and the taking and passing state-administered tests as required. I understand that I must pay the appropriate fees and attend the entire course in order to obtain a training certificate from Leaaf Environmental, LLC (Leaaf). I also understand that Leaaf’s training certification is not a replacement for the state certification and that I am not considered certified to perform work within the state until that certification is issued by the state and received by me. I understand that the state regulations require that I provide proof of certification if requested by the state agency.
Leaaf makes no representation or guarantee in connection with the materials in this course including the presentation made by the instructor(s). Leaaf hereby disclaims any liability or responsibility for loss or damage from violation of any federal, state or municipal regulation, ordinance, or law, resulting from the direct or indirect use of course materials. Mention of any trade names, commercial products, manufacturers or business logos contained in the material or mentioned by the instructor does not constitute an endorsement by Leaaf.
